Policy and Fee Schedule
The Miami County Dental Clinic is not a free clinic, however we do offer a sliding fee schedule for services which is dependent upon your household income and family size.
Patients should bring proof of income – 3 consecutive pay stubs, social security statement, child support, disability statement, etc. If applicable, please bring your insurance or Medicaid card.
We support those on Medicaid, the underinsured/uninsured and patients of all age groups.
New Dental Patients
If you are a new dental patient with the Miami County Dental Clinic, we encourage you to print and complete our forms prior to arrival for your first appointment. This will help you to speed up your visit!
